Adult Volunteer Waiver 2025/2026

Background Check Information
All of our volunteers 18 years and older that are not currently enrolled in high school or college MUST provide a background check. We suggest using www.directscreening.com, please select the national option. You may use your own company, at this time you are responsible for the background fee about $17.00. If you are a teacher, you can give us a copy of your id, since you needed a background check.
Photography & Behavior Policy
At NO time are volunteers to take pictures/videos of any child at our events with your phone unless you are our photography or assigned the job of photographer at an event. There will be no cigarette/vaping at any of our events. You are NEVER to leave with a child or escort them into the bathroom. If a child is having a BEHAVIOR, please walk away, their parent should be there at all times, when trying to help you could accidentally make it worse. Do you agree to EVERYTHING listed above?
Liability Waiver
In consideration for my participation, I hereby for and our respective heirs and successors promise not to sue and forever release SE4SK A NJ NONPROFIT CORPORATION, the owners of 22 Dickerson St Woodbury NJ and EVERY venue that we hold an event at during the 2024/2025 year, its officers, directors, board members, shareholders, employees, contractors, and volunteers both youth and adult from all liability resulting in damages or injuries incurred as a result of participation including those resulting from acts of negligence.
Photo/Media Release
I am aware that individual and group photos/videos are taken from time to time and in considerations for my participation I hereby grant my permission for such photos or videos to appear on Jami Saraullo, SE4SK's Facebook, Insta, and Twitter, etc. Along with venues that host events for us, or professional volunteers (photographer, face painters, magician, etc) this also includes online and paper publications used to promote SE4SK and recruit donations. I have read and understand this waiver of liability and photo release and I voluntarily affix my name in agreement. Please print your name below.
